A Comprehensive Program for Improving Health Care Outcomes in the Medicaid Population

A Medicaid Managed Health and Special Needs Plan was looking for strategies to engage members and their physicians in improving evidence-based care delivery and disease self-management. Over a four-year period, five campaigns were launched to distribute a number of health plan branded patient tools:

Over 10,000 seniors were targeted with the Senior Health Organizer to support senior health promotion, disease/condition prevention and screening through application of the U.S. Preventative Service Taskforce and RAND ACOVE guidelines—in coordination with Medicare’s Welcome to Medicare program for new Medicare enrollees.

Diabetes and Heart Disease evidence-based care adherence and daily self-management were supported through the distribution and promotion of the Diabetes and Heart Disease Organizers.

A health plan-wide campaign to support medication adherence and polypharmacy was also supported through deployment of  the Personal Medication Organizer.

To address the problem of unrecognized depression among members of all ages, a depression recognition campaign was launched to support depression recognition, screening and evidence-based care through use of the Depression Recognition and Management Tool.

All tools were reviewed and approved by the Minnesota Department of Human Services for health literacy and guideline adherence.
