News Case Studies Promoting Evidence-Based Care and Self-Management for Coronary Artery Disease(CAD) and Heart Failure (HF) in a Blue Cross Blue Shield AffiliateA leading east coast Blue Cross Blue Shield Affiliate aimed to reduce variation in the delivery of American College of Cardiology/American Heart Association standards of care for members with CAD and HF across commercial and Medicare business units. The following steps were taken: Claims data were reviewed to identify members with Coronary Artery Disease and Heart Failure The Heart Health Organizer II (Coronary Artery Disease) tool was customized and branded for delivery to over 20,000 members with Coronary Artery Disease. The Heart Health Organizer III (Heart Failure) tool was customized and branded for distribution to over 10,000 members with Heart Failure. An implementation plan was developed and launched to distribute the tools and coordinate the campaign in partnership with network physician practices.
