CCP Program Now Accredited for Continuing Education Hours for Physicians and Pharmacists

September, 2008. Today's physicians and pharmacists are leading implementation of patient-centered medical home models and integrated care teams to improve chronic care and health care performance. To prepare physicians and pharmacists for success in the new health care environment, the CCP program is now accredited for 40.0 CME and ACPE hours for physicians and pharmacists in partnership with Chicago's Academy for Continuing HealthCare Learning.

HSI Partners with Disease Management Association of India (DMAI) to Offer CCP Certification Program

 

September, 2008. HealthSciences Institute will partner with the Disease Management Association of India (DMAI) to support the application of disease management strategies, knowledge and tools in India--beginning with delivery of the CCP certification program in that country in 2008. HealthSciences Delivers CCP Program to Multidisciplinary CVD Program Team at Kaiser Permanente

August, 2008. HealthSciences delivered the Care Professional (CCP) certification program to nurse practitioners, physicians, case managers, dietitians and pharmacists at Kaiser Permanente's Georgia Region in July in coordination with the launch of the region's new Cardiovascular Disease (CVD) Management program. The CVD management program aims to improve evidence-based care delivery, chronic condition self-management, and shared-decision making for members with CVD disease, heart failure, diabetes or hypertension. Motivational Interviewing Video Training Series to be Added to CCP Certification eLearning site in 2008

June, 2008. HealthSciences will offer streaming video content on motivational interviewing foundations and patient behavior change scenarios in health care settings through an agreement with Dr. William Miller, founder of motivational interviewing. The video training series was developed by Drs. William Miller and Stephen Rollnick and is designed to offer practical instruction and guidance on applying motivational interviewing in health care settings and incorporating common health-related behavior change strategies and interventions. The content will only be offered to CCP program registrants.

Chronic Care Professional (CCP) eLearning Certification Site Upgraded and Expanded

May, 2008. HealthSciences significantly upgraded and expanded the CCP eLearning program. The new self-directed online program features Adobe Flash Player-enabled learning with audio. Additionally, access to the CCP eLearning program has been expanded from six months to one year. HealthSciences will be adding new learning resources, professional collaboration, and knowledge-sharing features to the site in 2008.

Fourth Edition of the Chronic Care Professional (CCP) Program Released

May, 2008. HealthSciences released the updated CCP program featuring substantially updated and expanded content on patient partnering, behavior change facilitation and health coaching practice and interventions. Additionally, evidence-based care guideline overviews for over 25 chronic diseases and conditions were updated for 2008.

HealthSciences Institute Supports National Sudden Cardiac Arrest (SCA) Prevention Program

February, 2008. HealthSciences Institute will deliver education and performance improvement planning sessions to US cardiology clinics and hospitals in several US locations in 2008 to improve prevention rates for SCA. The SCA prevention program is sponsored by Medtronic.

CCP Program Offered at the Three Leading Disease Management & Case Management Events in US

January, 2008. HealthSciences has agreed to deliver the CCP program as pre and post-conference sessions at the three major DM and case management events in the US. Please see the events page for registration details.

DMAA & HealthSciences Release First Health & Disease Management Professional Development Survey

November, 2007. DMAA & HealthSciences Institute released the first health/disease management professional development survey. The survey assesses current employee hiring requirements, orientation, and ongoing development programs. Core population-based management professional competencies and continuing education needs are also being evaluated for updating the CCP program.

Office of Vermont Health Access (OVHA) Selects CCP Program

September, 2007. HSI was selected to deliver the CCP program to Office of Vermont Health Access (OVHA) Staff in Williston this month. The OVHA manages all publicly health insurance programs in Vermont, including Medicaid and the State Childrens' Health Insurance Program (SCHIP).

Case Management Society of America and HealthSciences Institute Plan 2nd Annual 2008 CCP Pre conference

August, 2007. Based on the strong response to the June 2007 CMSA CCP pre conference, CMSA and HSI announced plans to offer the CCP program as a pre conference at the CMSA 2008 annual conference in Orlando.

HealthSciences Institute Delivers the CCP Certification Program at CMSA Annual Conference

June, 2007. HealthSciences Institute delivered the Chronic Care Professional (CCP) program at a pre conference session at the Case Management Society of America's annual conference in Denver. Professionals from some of the largest disease management, health plan, military, and public health organizations in the US were represented.

HealthSciences Institute Receives Funding from the State of Wisconsin to Deliver the CCP program in Wisconsin

January, 2007. HSI was selected to deliver the CCP certification program through a partnership with Marshfield Clinic's Medicare Group Practice Demonstration program, Wisconsin counties, the Medical College of Wisconsin and Security Health Plan in a pilot program targeting community-dwelling vulnerable elders.

HealthSciences Institute & Wipro Partner to Improve Physician Practice Performance

November, 2006. HSI announced a partnership with Wipro of Bangalore, India to deliver physician practice performance improvement program services to U.S. health plans and pay for performance programs. HSI developed a physician practice profile (MDP3) to benchmark physician practice performance and identify opportunities for practice improvement. See press release.

Calgary Health Region and the Alberta Health Ministry Selects the CCP Certification Program

September, 2006. The Calgary Health Region selected the CCP Certification program for delivery to nearly 100 health care professionals in the Calgary Health Region and other Alberta Health Regions. Calgary Health Region has been recognized by the World Health Organization as one of most advanced chronic care collaboratives in the world.

Horizon BCBS of New Jersey Delivers HSI's Coronary Artery Disease Patient Self-Care Products to 30,000+ Members

August, 2006. Horizon BCBS of New Jersey selected HSI's Coronary Artery Disease and Heart Failure Patient Self-Care Tools for delivery to over 30,000 members to increase adherence with evidence-based care medical treatment and support patient engagement and self-management.

Ontario Health Ministry Funds HSI Program for Toronto East General Hospital System Family Health Team

April, 2006. The Ontario Health Ministry funded an HSI program delivered to family practice physicians in Toronto implementing the Province's Family Health Teams (FHT) model of primary care. The FHTs are leading the redesign of Ontario's primary care system through multidisciplinary teams, technology, practice process improvement, and patient self-management support. Province-wide over 250 physicians and 225 other practitioners are participating in FHTs, serving an estimated 500,000 patients.

HealthSciences Institute, University of Toronto Medical School & Toronto's Centennial College Professionals Launch CCP eLearning Program.

September, 2005. HSI, in partnership with curriculum & health care eLearning design and competency development professionals from the University of Toronto Medical School and Centennial College, including Toronto's Robert Luke, Ph.D., Learning and Development, released the first disease management eLearning program. The asynchronous learning program is designed for distance for delivery to health care professionals who wish to complete CCP certification.

HSI Supports Largest Heart Failure Registry: ImproveHF

June 2005. HSI was selected as a partner on the ImproveHF heart registry--the largest outpatient heart failure registry to date. The study is aimed at improving adherence with American College of Cardiology guidelines for heart failure among nearly 200 US cardiology practices. Over 50,000 patients are involved in the study. Dr. Blake Andersen, HSI CEO will be facilitating national performance improvement sessions in Chicago, New York, Dallas, San Francisco and Los Angeles in partnership with leading heart failure experts. The study is funded by Medtronic and is co-chaired by Dr. Gregg Fonarow, FACC, FACP of the UCLA Medical School and Clyde Yancy, MD, FACC, FAHA, FACP, of Texas Southwestern Medical Center.

HealthSciences Announces Partnership with the Case Management Society of America (CMSA)

February, 2005. HealthSciences Institute announced a partnership with CMSA to offer the CCP certification program to CMSA's 10,000+ members. The CCP program will be provided as an advanced offering in CMSA's new Educational Library.

Version 3.0 of the Chronic Care Professional Program Released

January, 2005. the updated version of the Chronic Care Professional (CCP) program was released, updating disease management content and expanding the focus on behavior change facilitation and health care coaching.

South Country Health Alliance Selects CCP Program

February, 2005. South Country Health Alliance, a leading Minnesota County-Based Health Care Purchasing group selected the CCP certification program for delivery to public health and social services staff from fourteen Minnesota counties.

Version 2.0 of the Chronic Care Professional Program Released

December, 2004. Following the State of Minnesota Chronic Care Demonstration Project, HealthSciences Institute released version 2.0 of the Chronic Care Professional (CCP) Certification program.

State of Minnesota Chronic Care Demonstration Project

October, 2004. HealthSciences Institute received funding from the State of Minnesota Department of Human Services to deliver the Chronic Care Professional (CCP) Certification program to public health and social services staff from over ten Minnesota counties in partnership with PrimeWest Health Services.

Chronic Care Professional Certification Program Announced

August, 2004. HealthSciences Institute completed the Chronic Care Professional Certification program in partnership with the HSI advisory committee and national health care stakeholders.

Chronic Care Competency Modeling Project Completed
