HealthSciences Institute has designed a suite of evidence-based tools to improve patient engagement and provider delivery of recommended care. Unlike traditional patient education materials, these tools are designed to activate both patients and providers.

HealthSciences Institute's patient self-care tools can help case managers, disease managers and providers improve key performance indicators. We have condensed the latest care guidelines and self-care steps to facilitate ready reference and tracking -- by patients and providers.

Our care tools can easily be inserted in our patient pocket folders -- also ideal for storing notes, prescription information, or other patient materials. All tools can be branded (front and back) with space available for ADA, language translation information, or disclaimer. To support health literacy, all tools are written at the 7th grade reading level -- with large text size for seniors with visual impairment.



Senior Health Organizer - RAND estimates that U.S. seniors receive recommended care only one-third to one-half of the time. This tool is designed to help seniors and providers track delivery of evidence-based preventative care services. And, by recognizing and treating common senior health risks such as polypharmacy, falls, depression, incontinence, and cognitive impairment, unnecessary hospitalizations and loss of independence can be prevented.
